2008 Auction Contribution Form

Avalanche-Center.org (CSAC) is a non-profit organization with 501c3 status. The value of contributed items should be tax deductible. Check with your own accountant to be sure. All contributors will be recognized here on the auction home page, on the entry for the item, and for the rest of the season on either the list of contributors (individuals) or the Avalanche Network (companies).

Please fill out the following form as completely and accurately as you can. If you do not know certain parameters leave them blank. All contributions will be verified before being added to the auction - we will be in touch shortly. And thanks!
